Insurance & Fees

I am an in-network provider with Blue Cross Blue Shield PPO and will file claims on your behalf. If you use another insurance, it may cover services for out-of-network providers. I can provide you with a superbill that can be submitted to insurance for possible reimbursement.

Please contact your insurance company prior to our first meeting to confirm coverage for either in-network benefits or out-of-network reimbursement rates.

Cost of Evaluations

The cost of an evaluation will differ depending on the unique situation and referral question of each person. Cost will be finalized after a treatment plan has been determined. Insurance does not typically cover every aspect of an assessment and there are typically additional out of pocket charges. If you will be paying out of pocket, I can work with you to set up a payment plan.
